Brief Answer:
3 possibilities

Detailed Answer:
There are 2 possible causes of such neurological manifestations-
1 would be a minor stroke. I will consider this 1st since its of a sudden onset and therefore the stroke is a first possibility
2 would be a possible delirium. This can come suddenly as well. However there are usually some preceding features or history that we might be missing in the short description above (in the query).
3 would be a possible seizure. This is also sudden, and the seizure might have been missed as he was sleeping. This may be post ictal confusion.
I would recommend testing a MRI brain, EEG and also electrolytes.
I hope that this will help.
Please feel free to ask for any clarification.
Best of luck,
Dr Mittal
