Question: elderly male with 96.1 temperature, had heavy sweating, blood sugar 449, 2 hrs ago, blood sugar now 289 and the sweating has stopped after Humalog insulin. History of diabetes, COPD, Congestive heart failure,

This temperature is lower than the normal body temperature of 99.5. This is probably why he is complaining of feeling cold and needs to be covered.

From the information you have provided, sweating is not a common symptom of high blood sugar. It is more common in low blood sugars and infections. If his blood sugars were well controlled in the past and this increase is out of the normal usual range, then there is most likely an infection going on somewhere. Low body temperatures are also suggestive of a possible severe infection.

I will not want to take any chances here. I will like him to be evaluated for infection. The lungs, urinary tract, skin, etc showed be investigated for possible infectious causes.

In all, I am concerned about an infection from all the information you have provided. Low body temperature, chills, high blood sugars. He needs to be seen by a physician for evaluation and management.

He has no cough, but is always short of breathe. And today has not showed any worsening of the breathing. For him he appears to be breathing normally. His blood sugars are and never have been controlled. He has started eating fresh fruits in his diet over the last week, which has lead to some higher blood sugars, I need to control the amount he is eating to help the blood sugars from spiking so high. Will this help?

If you think he is stable, then we can buy some more time by observing a little longer to gather more information.

Try and avoid excess fluctuations in his blood sugars. The goal here is not really to have ideal values but to avoid low sugars under 100 and high sugars over 150.

Follow up with me in 12hours. Tell e what the temperature and blood sugars are. Also, mention about his general state and whether there are any new symptoms or not.
