Brief Answer:
Possible worms,further lab tests suggested as following...

Detailed Answer:
Hi,

I have gone through your medical history carefully and understood your concerns.

The itching around your anal area, sometimes extending to vaginal area, especially worst at night is suggestive for possible intestinal parasites (worms).

I suggest to send your stool at the lab to get examined for ova parasites.

There is another method to detect worms: before going to bed, put a piece of scotch tape on the anal/perianal area for all night long. Once you wake up in the morning, take it off and put it on a lab glass. Discuss this further with your doctor.

As treatment, I'd suggest;
- mebendazole or albendazole with repeated dose within 2 weeks
- take anti-itching meds like anti-histamines
- take probiotics

Hope it was of help!
