What Causes Heart Pounding And Varied Blood Pressure Level While On Chlonidine?

Question: I am taking Chlonidine, hydralazine and carvedilol and am having bad reactions... a hard pounding heart and faint headness. My blood pressure goes up an down over the period with the systolic rate high and diastolic rate low. The heart rate remains low and fairly steady. The periods are: 2 (25mg)hydralazine and 2 (.01mg) chlonidine (at 4Am, 12PM, 8PM) on 8 hour schedule, the carvedilol (at 9am and 9pm) is on a 12 hour schedule. At one point, the carvedilol (2- 6.5 mg) is only 1 hour separated from the other two. Are these medicines compatible? Should I reduce the dosages by half? I started these medicines last week and was hospitalized for two days, starting this new regimen on Tuesday. I had a bad day on Wednesday and then a good day on Thursday when I reduced some of the dosages. Friday was a bad day again. The bad symptoms seem to start in the early morning at about 1:30 AM, which led me to think of the conflict between carvidilol and chlonidine at 9 and 8 pm. What do you think? I have bad kidneys, which are part of the problem. I am not on dialysis. I am also taking a 1 mg bumetanide pill in the AM. Furosemide does not work well with me.

Clonidine is unlikely to be the culprit drug. It is more likely to be Hydralazine.
Clonidine has a therapeutic window phenomenon , that is in blood concentration of only 0.2-2.0 ng/ml the blood pressure is lowered. Further increase in blood concentration does not cause fall in blood pressure.
Hydralazine acts by causing arterial dilatation. So if the dosage is more its likely to cause low blood pressure especially diastolic.
I would suggest you to lower the dosage of Hydralazine. If the symptoms persists, would suggest you to consult your physician.

Thank you so much for your reply. I think you are spot on and I did reduce the hydralazine to 25 mg per 8 hr. I also adjusted the space between the clonidine and coreg in the evening and morning doses. This seems to be working as I feel much better today. I reduced the dose of coreg too, to 6.5mg each time rather than 13 mg each time. So far, so good. The pressure is staying at a reasonable level so far.
