Brief Answer:
Proper investigations and consultations

Detailed Answer:
Hi.
Thanks for your query and an elucidate history.

(To recapitulate: 52 / Female - 5 kids - urgency - frequency - small amount - 20 times today - taking Azo. - not visited Doctor.)

I would like to ask just a few more information:
Since when have you got this problem?
Any evidence of uterine prolapse?

The most probable causes are:
- This looks to be a problem of Urinary tract infection, the basic cause may be uterine prolapse.

I would advise you the following:
-First of all, send the urine sample for routine, microscopy, culture and sensitivity before start of antibiotic. This is the most important step as a single tablet of antibiotic can change the reports.
-Clinical evaluation and actual physical examination including per-vaginal and per-speculum by the Gynecologist to see for uterine prolapse, urethral opening, tenderness in the pelvis and so on.
-Further tests of blood for sugar, urea and creatinine and as per orders of your Doctors.
-Ultrasonography of the whole abdomen including per-vaginal
-Institute the proper antibiotic cover till getting reports.
-Further tests of CT scan of the abdomen and Intra-venous Pyelography only if required by your Doctors.
-Second opinion of an Urologist.

All these things together will get you the most proper diagnosis in all respect and give a plan for further management.
