What Causes Excessive Gas Production, Belching And Flatulence?

Question: I am having excessive gas production: belching and flatulence. Belches taste bad and have horrible odor. I am also experiencing very watery diarrhea. These symptoms occurred @ 5 hours after consumption. This is the third occurrence in four weeks. I was diagnosed with gastro-enteritis due to antiinflamatories about 6 years ago.

I appreciate your concern

Anti inflammatory medications like corticosteroids or NSAIDs are associated more commonly with Peptic ulcer disease rather than Gastroenteritis. This can be prevented by administering PPIs along with the Anti inflammatory medication. Empirically a course of PPIs like pantoprazole along with sucralfate , bismuth subsalicylate can be given to adequately treat the flatulence and belching. If there is evidence of infectious diarrhea such as blood or wbcs in stool, fever then antibiotics can be started including a quinolone like ciprofloxacin with metronidazole. If there is no evidence of infectious diarrhea then fluid replacement and IBS ( irritable bowel syndrome ) can be considered.

IBS involves spasmodic pain and in some patients is associated predominantly with diarrhea and in others with constipation. Mebeverine and combination medication such as EUCARBON can be useful in this regard. The bad odor can be due to a number of factors. GIT Malignancies should be ruled out if empiric treatment fails to improve outcome. If the symptoms persist then an upper G.I endoscopy can be planned to look for any abnormal growth or an associated hiatal hernia which can be contributing to these symptoms. Blood counts and metabolic profile in addition to an ultrasound would also be required as part of your workup.
