Question: My boyfriend has Diabetes and can’t “get it up”. Will you give me a detailed explanation as possible please? He also has gastric reflux, IBS and is waiting on a fundoplication? An explanation for his e.d would be much appreciated

Most common cause if ED is diabetes only. It develops after many years of uncontrolled diabetes.
However uncontrolled diabetes at an early age can also cause ED.
I would like to know his age and since when is he having ED.
Also want to know his blood sugar levels.
Also I would like to know that is he having spontaneous morning erections? Ask him and tell me.

He was thirty one when he last had intercourse. He is now fortyone. He gets spontaneous erections in the morning for a few seconds, sometimes for a few minutes. He drinks alcohol, a bit too often and has too many units, he controls his diet quite well, but is waiting on a fundoplication....he always takes his insulin, and has a throwaway pen....

Since he is already having morning erections means still he has capability for erection.
Get his color Doppler of penile vessels.
In such situation I advise following :
1. To take Methylcobalamine supplemtation.
2. To use Sildenafil as on when required basis that is whenever sex is desired.
3. To cut down his alcohol as this would cause his diabetes uncontrolled as well as reduces erection when sex is tried after consumption of alcohol.
4. To keep his blood sugars well controlled.

He took viagra etc and said it was like it was on fire. He injected in to it and refuses to do it again as it caused severe pain and bruising. I said he had done it wrongly but he said that he had done it correctly.

If his cardiac status is normal he can take Sildenafil. Ask him to take oral preparations available instead of injectables. He should not mix it with alcohol.

I don't think getting it prescribed should be a problem for you.
