I have read through your query in detail.
1. ESR is erythrocyte sedimentation rate and is a test that indicates inflammatory activity in a person. If it is high then there might be a disease process in that person. It only tells that a disease is there but will not say what disease it is.

2. The normal range of ESR in a young adult male is 0-22 mm/hr. If you have ESR 23 mm at end of first hour then it is upper limit of normal. This is not a concern.

3. Fatigue is a general symptom and can be present in mild weakness and fever and also in serious conditions like infections and cancer. If you have fatigue then it is important to get general health check done by a physician.

4. Cancer causes symptoms like sudden unexplained weight loss, decreased appetite, swellings in the body, vomiting or diarrhea for more than 2 months, continuous fever not responding to medicines in most people.

5. If you have history of cancer in your family and want to get checked for cancer then please attend cancer screening clinic at a cancer hospital. They will do all required screening tests for cancer including detailed blood test, Chest X ray and ultrasound scan abdomen.
