Question: What would cause burning, needle and pin sensations when startled (any sudden noise or action ) and tremors when walking.

I am Dr.Ajay Panwar,a neurologist,here to answer your query.

If you would have presented to me as an outpatient,I would have told you that the startle paresthesias(burning,pins and needles)you are having is likely a physiological enhanced startle response due to anxiety.

Further,you are established to have spine degenerative disease with cervical canal stenosis along with radiculoneuropathy.These conditions are characterized by burning,pins and needles paresthesias,which can be intermittent initially.These are adding-up to cause your symptoms on exposure to a startle stimulus.

Regarding tremors,I would like to know a few details-
1)You said tremors occur on walking.Are they present only on walking or sometimes otherwise as well.
2)Do you have tremors in upper limbs,lower limbs or all four limbs?
3)Are they symmetrical on both sides or asymmetrical?
4)Since when you are having them?

Lastly,please upload your low adrenal report in the follow-up so that I can have a better impression although symptoms don't appear to be due to low adrenals.

I meant to type upon waking. it can be after 15 mins of light sleeping or in the morning. immediately following wake, I get these inner body tremors.
the adrenal report I do not have access to. I can tell you the lab said adrenal fatigue and you can see the endo notes below.
the pins and needles come and go quickly almost like the fight or flight adrenaline rush when a cop cast runs up behind you but now I am flying it all the time with even a sudden noise

Thanks for being in follow-up and providing further details.

I have gone through the medical document you uploaded.It also says you have non-toxic multinodular goiter.What about your Thyroid function tests?Are you on any medications for thyroid disorder?If yes,please tell details.

Flight or fight response is diminished in hypoadrenalism(low adrenal functioning).So,it should not behave as exaggerated here.

Still,the cause of these symptoms appear to be cervical stenosis/spinal degeneration with accompanied radiculoneuropathy.These symptoms tend to be intermittent with this condition and as the disease progresses,the symptoms become persistent.It is not rare,to have presenting these symptoms like a startle.

Inner body tremors are probably resulting from anxiety.

Please follow-up with details of thyroid function tests and thyroid medications.

Considering all the medical details,I hereby suggest you to get evaluated for the cause of neuropathy.The following initial investigations should be planned-
1)Blood sugar-fasting and post prandial
2)Nerve conduction studies
3)ESR

Meanwhile,you should be on a typical management protocol like-
1)Tab.Pregabalin and methylcobalamin combination
2)Calcium and vitamin D supplementation
3)Regular physiotherapy/spinal exercises
