What Causes Burning And Painful Skin Eruptions When Suffering From Herpes?

Question: hi,
my nanny is 85 years old.She is suffering from blisters around left armpit, left breast and back.

She has been identified as herpies but even after one month consultation,we see no results.She has been medicated with medicines below:
Cap Spinal Care
Tab Jomdec
Tab Sensevial
Tab Dolopal
Injection Dynapar J amp.
But Still she is suffering from burning eruptions and pain.Please guide me if I need to consult Dr again because it is not increasing ,only pain is there.Its difficult for her to walk and go to Dr at this age.

Hi,

Firstly as we were not aware of the disease,My mother applied Safromycin cream and boric powder.

Then one of the family person gave "LCZ-Mont","Moxclav625" and "Omeprazole" and "Flexon" tablets for 15 days.

Then we consulted a renown Dr because of blisters still spreading and we got to know its harpies(as he said).The medicines prescribed are mentioned in first mail.

I need you to guide me:
1.As she is really old and difficult to get to Dr,can we have her medication at home.We are not aware whether it is increasing or not.Visibility wise,we do not see any elation.

2.We have been applying safromycin,Lignocaine Hydrochloride Gel to her.

3.If any special diet,she need to maintain.

Hello

I would keep a possibility of Post-Herpetc Neuralgia (PHN) which is defined as pain persisting more than 1 month after herpes Zoster/shingles. Shingles is characterized by painful, fluid filled vesicles, usually localized to a particular area only. The lesions usually resolve within a week or 10 days, however individuals can continue to have pains, burning, stinging and stabbing sensations, long after the episode of shingles has healed. This is known as Post-Herpetc Neuralgia (PHN).

This is a very common complication in older age group, not so very common in young population

The pain is due to inflammation and scarring of nerves affected by the episode of shingles. Treatment is effective but sometimes can lasts months or years, specially in old age group of patients:

Treatment options are:
-NSAIDS
-Tricyclic antidepressants(Amytriptyline, nortriptyline)
-Gapapentin/Pregabalin
-Carbamazepine
-Topical Capsaicin gel/cream
-Topical Lidocaine gel/cream

I usually treat my patients with a tricyclic antidepressant once daily at night (sensival 25 mg, which she is already taking) along with Gabapentin 300mg twice or thrice daily. The dose of tricyclic antidepressant (sensival tablet) can be increased to twice daily OR thrice daily depending on the response.

An oral Vit B12 Methylcobalamine in doses of 500 mcg to 750 mcg is also useful for neuralgic pains.


Gabapentin and methylcobalamin are available as a combination, Cap. XXXXXXX M75. She can take it twice daily along with tablet Sensival 25 mg twice daily.

As these are prescription medicines, therefore I would advice that you see a dermatologist and get them prescribed.
