Question: Hi Doctor, I am 30 years old. I am facing lot of trouble concentrating and maintaining focus. Even when somebody talks to me, I sometimes find it difficult to follow the conversation and comprehend what is being spoken. I tend to forget things even after a deliberate and conscious effort to remember them, even small things which I used to easily remember. When I speak about something, I find it difficult to collect my thoughts and speak in a mature and organized manner. I tend to speak in a staggered manner, finding it difficult to find the right words and compose what I speak. The complicated things at work which I used to very easily understand and manage are becoming increasingly difficult for me. Even when I watch a movie or a TV program, I find it difficult to follow the conversations and events properly. When I read something, I find it difficult to understand the gist of it and tend to focus on unnecessary details. All these I used to be able to do very effectively and effortlessly earlier, and now I am unable to do these properly and it is impacting my work and also personal life. Please help me in diagnosing the root-cause and availing the right treatment. FYI, I have recently had an MRI scan as suggested by Neurologist Dr. XXXXXXX XXXXXXX and it was all normal. I have taken a 3 month course of Piracetam but it did not seem to help to a great extent. Regards, XXXX

Hi, Please find below my responses to your questions: 1. How is your mood these day? Mood is Normal 2. Do you have any anxiety while talking to people? Not sure if it is anxiety. But just the fear of not following what people are saying to me. It may be causing more problems than what already exist. 3. Did you have same problem while school time also? Maybe sometimes I had. 4. Do you any anxiety symptoms throughout the day? I am not sure 5. What do you do and how much attention your work need? I am an IT employee. My work demands good amount of attention. 6. Do you have any family problem these days? No 7. Are you able to driver properly? Yes 8. Are you facing any difficulty while reading? Not while superficial reading as such. But while comprehending, Yes. 9. How is your appetite or sleep? I think appetite is not completely normal, but sleep is probably fine. 10. Any history of alcohol or other addiction? No

Brief Answer: You may have anxiety disorder Detailed Answer: Hi I would keep possibility of underline anxiety disorder. Sometime, due to anxiety, people face difficulty in maintaining concentration. That may be the reason in your case. You can try any antidepressant like paroxetine. It would help to decrease the underline anxiety. If you were mine patient and came to my opd, then you would have been suggested tab paroxetine 12.5mg at night. This medicine would take two weeks to show response, so you can take this drug after psychiatrist consultation. You can also try relaxation exercise like JPMR or deep breathing to keep you relax.
