I read your question carefully and I understand your concern.

That type of symptom you describe, feeling heartbeat in the ear is otherwise called pulsatile tinnitus. It can have many possible causes, mainly issues with the blood vessels. Such causes may also include general conditions anemia or high thyroid function which can be excluded through simple blood tests, but are unlikely since should cause tinnitus on both sides. So more likely a local cause.

Atherosclerosis of the blood vessels, causing turbulence and increased speeds of blood flow is one possible cause, common at your age. Some types of blood vessel tumors of the neck or skull arteries also are possible vascular causes.

However one other origin may also be increased awareness, due to conductive hearing loss making you more sensitive to the inside sounds like blood flow. Fluid in your middle ear or a perforated eardrum may be such causes and could well be the origin considering your infection.

So I believe the possibility of an infection complication should be considered first. If that is ruled out and symptom is persisting, then an imaging study like angio MRI may be scheduled to rule out major blood vessel related causes. Whether you need additional treatment or not depends on the cause.
