Question: Hi I have recently moved on to paxidep cr 12.5 from cipralex 10mg at the direction of the Doctor .also continuing with spenzo 0.5 mg . It has bn only one night that I have taken the medicine . I was on paxidep 25 and oleanz 6.5 four yrs back and I remember I had slept for days at a stretch when I started the med . After that i was on paxidep 25 for 3 yrs when i changed my doc as I wanted to gradually wean of medicines . I was initially diagnosed with panic disorder along with depression . I must say paxidep 25 worked wonders for me and moving to escitalopram was not so effective in controlling my panic disorder in the last 2 months especially . Recently I also have the problem of sleep with the entire night filled with vivid dreams and not waking up refreshed . I have been missing work because of this and have been on and off clonazepams which induced a lot of drowsiness rather than fresh sleep .

My doctor now said paxidep will help me sleep well .

I want to know if paxidep will now help me sleep , will I have any withdrawal symptoms of cipralex since my doc didnot taper cipralex and put me on paxidep directly .

Wat are the side effects I can experience from paxidep ? And how long before I can see changes taking place

I can understand your concern. Paxidep is Paroxetine and is a SSRI drug. Cipralex or Escitalopram too is a SSRI drug. Both have almost same mechanism of actions. So shifting to Paxidep from Cipralex would not cause any withdrawal effects. So no need to worry.

Yes, Paxidep will help to sleep. The drug causes sedation and will help in refreshing sleep.

Side effects like nausea, dizziness, constipation, dryness of mouth, blurred vision etc are seen commonly in high doses. But in therapeutic doses these side effects are mild. Delayed ejaculation is also seen in some individuals but after prolonged therapy and this side effects is reversible so no need to worry for that too.

My sleep is filled only with dreams .is there anything to worry . I keep researching on such things and worry a lot . Ofcurse I don get refreshed sleep because of these dreams which are not nightmares but random dreams .

For some time till the drug start acting, you can continue to take Spezo 0.5 as advised. The drug will help in sleep and in the mean time you will become stabilised with Paroxetine. The nightmares will reduce with Paroxetine.

One last question . Since I was on 25 mg paxidep for 3 yrs , is it necessary to go back on the same dose because similar problems don't persist anymore especially there is no depression but only anxiety disorder which is controllable with 10 mg dose . I have bn on a 10 mg dose for a year now and have not ever felt that the dose needed to be increased . I have had minor panic attacjs on 10 mg because of mostly stress and excessive worry which was completely not there on paxidep 25 mg . Since I want to wean off meds I feel 25 mg for 3 yrs and 10 mg for another year is good enough for me to continue on a milder dose . What do you suggest ?

Yes, since anxiety is milder this time and depression is not there so in such case you will not need the same 25 mg dose. In my opinion 12.5 mg controlled release tablet would do the work. This will control anxiety and will help in sleep too. No need to worry.
