Brief Answer:
Prostate biopsy is the next step.

Detailed Answer:
Good Day and thank you for being with Healthcare Magic!

a felt prostate nodule on digital rectal examination is an indication for prostate biopsy. Some would do MRI first to check if the nodule would appear as an active nodule prior to do biopsy. That being said a nodule in the prostate is always considered cancer until proven otherwise.

Brief Answer:
12 samples are taken minumum.

Detailed Answer:
In a biopsy it can be done under local anesthesia or IV sedation. It is a transrectal ultrasound guided biopsy and minimum of 12 samples are taken through out the prostate to determine if prostate cancer is present or not. The biopsy would determine if you have cancer or not and not just a screening tool to detect cancer.

Good Day and thank you for being with Healthcare Magic!

Prostate cancer is still possible even with a low PSA (e.g sarcomatode cancer) but it is unlikely and rare prostate cancer variants. Nodules could also be calcifications in the prostate and they can be also felt as hard nodules on DRE.

Complications of prostate biopsy would be blood in the urine and blood in the semen that can last upto 3 months. The most dreaded would be fever and sepsis which can be life threatening if not promptly treated. This happens in 1-10% of the time and can be prevented by proper antibiotic prophylaxis.

In the end everything would be speculative and the only way to rule out prostate cancer is to do a biopsy
