What Are The Repercussions Of Taking Rabies Vaccination In The Wrong Order?

Question: Question to : Dr. XXXXXXX Taher XXXXXXX

Hello Doctor,
Situation is like that.
I took My First Vaccine on 3rd December and Second vaccine on 6th december 3rd vaccine on 10th december and mistakenly because of my Ignorance I took one more vaccine(4th vaccine) on 11 december.

But on 8th day(wrong day) (11 December) I took vaccine at 2 pm but after 4-5 hours(arround 7 pm) I got again Exposure to Rabies( Dog lick my scratch). So My question is that,

a). Doctor since Im expose to rabies on 11 December again after 5-6 hours of vaccine on the same day. So what should I do now?? Should I continue for same schedule even after new exposure on 11 december?? Or I should start vaccination from the begining??

b). as I took 4th vaccine on wrong date. will it cause any bad effect? is it okii to take 4th vaccine on 17 december now??

I took 4th vaccine on a wrong date (11 december), So Now I will take 4th vaccine again on 17 December and Fifth Vaccine on 31 december.

So, I have taken vaccines in this way:-

0 day( 3rd december):- First vaccine
3rd day(6 december):- Second Vaccine
7th day(10th december):- Third Vaccine
8th Day(11 december):- Forth vaccine(Mistakenly I took it)

Pls Guide... should I start my vaccination from begining or I should continue the schedule??

I have gone through your query diligently.
Sorry to know that you are exposed to dog bites again.

Since you have already taken 3 doses of vaccine on day 0 (3rd Dec), 3 (6th Dec), 7 (10th Dec) and an EXTRA DOSE by MISTAKE on 8 (11th Dec), these doses are sufficient enough to produce adequate antibodies against rabies.
But unfortunately you had another exposure (scratch licking) on day 8 (11th Dec) 4-5 hours following the EXTRA DOSE by MISTAKE, what you need now is complete the vaccination as per the schedule on day 14 (17th Dec) and 28 (31st Dec).

If the culprit dog is found to be healthy after 10-14 days following the exposure on 11th Dec., you can stop taking the last dose on 31st Dec.

Pls answer These Queries:-

a). If a Dog Bite a person During the vaccination of ARV Than what to do? (During a vaccination means after 2nd vaccine or third vaccine) Should that person start the vaccination again?
Should a person restart His ARV course from the Begining (like from day Zero)?? Or there will be no change in vaccination schedule?

Note:- If Person is Not Vaccinated by ARV before.(Means taking ARV course first time)



b). If a Dog Bite a person after 5 days of His 2 Booster doses( day 0 and day 3) (person has already taken ARV course 24 months before)than what a person should do? should he take vaccine again or booster doses again required? or what? pls explain...

Note:- if a person has already taken ARV course 2 years back.

1. He should take the ARV as per the schedule.
2. The two booster doses taken on day 0 and 3 are sufficient enough, there is no need to take vaccine again. Since with two booster doses produce sufficient amount of antibodies against rabies, hence there is no need to take another booster dose.

1). Is there any test for check the Rabies Antibody Titer Test in human?? Pls explain this Test.... what is the cost of this test)approx)? is this test can b done in small pathologies or In big pathologies??

Is this test is necessary for everyone or its necessary in special cases??
After how many days of Rabies Vaccination this test can be done? can this test be done after 3 vaccines of rabies or after 5 vaccination of rabies??
pls explain....

2. How long AntiRabies Vaccine take time to develop antibodies in human??

1. Rapid Fluorescent Foci Inhibition Test (RFFIT ) measures the ability of rabies specific antibodies to neutralize rabies virus and prevent the virus from infecting cells in human or animals. It is both sensitive and specific in the hands of well-trained laboratory personnel.

These antibodies are called rabies virus neutralizing antibodies (RVNA) and are used to evaluate an immune response to rabies virus antigens.

Usually it is not required for everyone, however it is necessary to monitor antibody levels in persons that may have an occupational risk of rabies virus exposure (e.g. veterinarians, hunters, rabies virus laboratory workers, etc.).

It is used to check the immune response of these high risks personnel undergoing rabies postexposure prophylaxis when major deviations in the vaccination schedule occur, or there are concerns about a patient's immune status.

This test is not done in small laboratories, it is done in higher centers like in XXXXXXX or XXXXXXX

Regarding the cost, it is better to contact those centres for more details.

2. As per the guidelines by World Health Organization (WHO) RVNA titre of 0.5 International Units per milliliter (IU/mL) is taken as an adequate response to rabies vaccination.

It tends to be highest approximately 2-3 weeks after completing a primary rabies virus vaccination series. After being vaccinated, antibody levels subside over time.
