Question: The area that is circled in the center, is that the annular tear? Also whats the area circled towards to bottom?

The circle at the top is the para vertebral fascia and fat. It is not annular tear.

The bottom circle is the tip of spinous process in the vertebra.

is the top area circled a tear? and whats the white area circled at the bottom? is that the cyccox bone. i have been having pain in that area. i have a tear and protrusion at L5 S1 disc.

The top circle is not a tear and is the fat in the paravertebral area.

The bottom circle if the spinous process which is a small bone projecting in the midline posteriorly from the lumbar vertebra.

This is at a higher level and the coccyx is not visualised in this image.
