Brief Answer:
Tinea; oral and topical antifungals

Detailed Answer:
Hello. Thank you for writing to us.

I have gone through your query abd i have reviewed the images.

I can see scaly patches on dorsum of hands and some scaling at palmar creases of fingers.
I will keep a possibility of fungal infection i.e tinea mannum.
Is it on both hands? Is it itchy?
If i was the treating doctor i would suggest treatment with oral antifingal e.g terbinafine or fluconazole tablets. In addition I would also ask you to apply a topical antifungal cream e.g terbinafine 1% cream twice daily.
These are prescription medication and should be used under supervision of a doctor. I suggest that hou visit a dematologist in your vicinity fir a confirmatory diagnosis and for the needful.

Brief Answer:
I suggest a KOH preparation made from a scraping as a confirmatory test

Detailed Answer:
Hi.

Tinea mannum is a fungal infection. Most common cause is trichophyton rubrum. This fungus is ubiquitous and is present in soil.
I suggest you to take a KOH preparation made by scraping a small amount of scale from affected area which should show fungal elements i.e hyphae under microscope. This would be confirmatory in case this is fungal infection. If this test shows as positive that would be confirmatory. Treatment would be with oral and topical antifungals.
However if KOH from scraping is negative then we would have to consider other possibilities like hand eczema.
Clinically this looks like tinea however tinea is usually unilateral i.e on one side and rarely on boh hands. Since you have it on both hands therefore a KOH preparation made from a scraping would be either confirmatory of tinea or rule it out.
