Suggest Treatment For Vertigo, Occipital Headache And Arm Pain

Question: 60 year old male BMI of 40 on omeprazole for two years for HH. Had elevated blood pressure Thursday 150/90 day before had bad case of vertigo. Dr vist complaint was the vertigo, elevated bp and dull achy pain in back of head along with jaw pain. Dr could not make me dizzy he put me on hydrochloride said to loss weight. Bp at time of office visit 140 /86 .Today first day of new med mild left arm pain tingling similar to My HH symptoms and still mild head pain. Walked today 45 minutes no shortness of breath same as yesterday. Thoughts?

I passed carefully through your recent medical history and would explain that your actual complaints (vertigo, occipital headache, etc.) seem to most likely to be related to hypertension.

Also, as you have a huge BMI of 40 (class III obesity) it is likely for some degree of backbone degenerative disorders to be present. In this regard, a careful clinical investigation through cervical-thoracic backbone X ray study would be advisable.

In addition, for ruling in/out any predisposing factor of secondary hypertension, it is recommended performing some lab tests:

- complete blood count (exclude anemia),
- thyroid function tests (thyroid dysfunction may trigger hypertension),
- renal and kidney function tests,
- blood electrolytes level (for exploring adrenal glands),
- fasting blood glucose and HbA1C (exclude diabetes),
- resting ECG (exclude possible cardiac ischemic changes),
- fasting lipid profile (a risk factor for CAD)

Meanwhile, I recommend to closely monitor your blood pressure values for a couple of days up to a week (several times daily) for a better review of your ongoing anti-hypertensive therapy.

You should know that Hydrochlorothiazide needs to be used regularly for a couple of days for showing its beneficial effects.
