Brief Answer:
Surgery is the best option.

Detailed Answer:
Hi!

Internal hemorrhoids also called as "piles" of the third degree mean a state in which they cannot be practically pushed back. There are many problems associated with this, including painful defecation, thrombosed piles and such issues.

I would suggest that the best option is to get these piles removed surgically.
With stapling, the recovery time and problems associated with surgery are very minimal.

Post operative painkillers can make this a smooth experience without hassles.

At this stage, medicines are most widely used only to lower the symptoms and not for any cure possible. Though there are many "almost magical" cures, mostly from Ayurveda and Homeopathy, which only complicate matters further, increase your suffering and are a total waste of time, money and energy.

I feel that after being presented with this information above, you might want to get them surgically removed.
