Question: My son has been experiencing sudden severe knee pain. He is 3.5 yrs out from a bone marrow transplant from ALL. Any idea what could be causing it. Transplant was from brother, 10 out of 10. No swelling or "heat" at the site.

Brief Answer:
Can be inflammation or muscle spasm.

Detailed Answer:
Hello,
Thanks for posting your query.
A sudden severe pain in the knee can be due to a muscle spasm or an inflammation of the knee joint. At his age, rest, hot compress and mild pain killers are the best way to treat it. Avoid the painful activities for a few days and use a pair of knee supports to decrease the abnormal movements.
Do watch for any swelling or increase in pain in which case an MRI scan of knee joint will be required to rule out any internal injury or hemarthrosis.
I hope this answers your query.

As I mentioned, no swelling. X Ray revealed nothing. Second occurance in 6 weeks. Pain is sudden and acute. Both times it has occurred while my son was asleep.
As an oncologist have you heard of these severe mystery pains? It is debilitating.

Brief Answer:
Can be bone pain.

Detailed Answer:
Hello.
Thanks for writing again.
Sudden bone pains can also happen after a marrow transplant. It is best to take mild pain killers and take rest to control these.
