Suggest Treatment For Severe Headaches

Question: Hi,

I am having headaches from last 1.5 months and have consulted 2 physicians.
The headache is generally on the right side of the head and the pain is almost all time during the day. Sometimes the pain is very high, and most of the time it is light headache. It is then followed by heaviness on the right side of eye till the back of my head. The pain is sometimes make me cry and it is very difficult to do daily activities.

Both physicians have told that it is Migraine and have given me Tablet Sibelium 10 mg and ETILAAM S 5mg.

I am continuously taking this at night for last 15 days. The pain is decreased but still having mild pain almost all the time of the day with frequent rise of the pain for 1-2 mins.

I was also advised to take Pain Killers Grenil which i have avoided and have only taken twice in last 15 days when it become unbearable.

Even after taking medicine for 15 days, i am not getting relief. Please let me know what to do?

Hello ,
I have gone through your question and understand your concerns.
Headache in young person is mostly primary and commonest being tension type and second commonest migraine.
The headache of migraine is usually hemicranial, throbbing nature, starts slowlty and peaks in hours associated with nausea, vomiting, photophobia, phonophobia.
The headache increases with exertion and decreases with rest and pain relievers.
Some person may have aura before onset of headache.
For treatment it is advisable to take proper timely meals, adequate sleep and rest.
Drugs like Triptans,beta blocker , calcium channel blocek(Sibelium), Topiramate, Di-valproate, amitryptylline may be tried .
The effect of drug may take 2-4 weeks for complete action.
If you are having improvement then continue the same drugs and wait for 1-2 weeks.
Take pain killers at the onset of headache and don't wait for peaking as the effect of drug is poor once the headache reaches peak.
Try the preventions as advised.

Hello Sir,

Thanks alot for your detailed answer.

My primary concern was that is this is actually Migraine. I have never had headaches like this before and i am not very much familiar with migraine.

I was terrified thinking whether this is Migraine or something else which is causing such pain in the head. It all started from the back of my head and neck and now captured my right side of the head.

If i even touch the area near the right side of the head, it is painful and it starts the pain on pressing it. It gives me a feeling that something is swollen or heavy on the right side creating pressure.

Will taking the prescription settles this migraine or is it going to be like this everyday. I am very much nervous.

Hello ,
If you are not having continous headache, intractable vomiting, visual loss, diplopia, hearing abnormalities, limb weakness, seizures then possibility of serious disease requiring investigations is less likely.
According to your description the most likely diagnosis is migraine.
I will advice you to continue drugs and consult again if not releived for change of drugs after 2 more weeks.
