Suggest Treatment For Severe Anxiety While Withdrawing Off Levosulpiride

Question: Hi doctor,
In case you recall, I had asked you a question regarding my case a couple of months ago. However, I could not really follow up on your advice since my health took a turn for the worse recently. At the time I messaged you, I was under the process of tapering off levosulpiride, which I had been taking for about 8 years (with doses varying from 50-300 mg). The reason for the withdrawal was that the levosulpiride (I had been taking 75 mg for a while) had caused a decrease in my testosterone levels and an increase in my prolactin levels beyond what is normal.
However, since I completely stopped taking levosulpiride (Dec 26, 2016) I have been experiencing severe anxiety for apparently no reason. I am experiencing symptoms of nausea, dyspepsia, nervousness, palpitations, muscle tension, with a severity that makes it almost impossible for me to function normally.
The psychiatrist I am currently under has put me on sertraline and clonazepam to combat these symptoms; and bisopropol 2.5 mg for the palpitations/increased heart rate in consultation with a cardiologist. However, nothing really seems to be helping at the moment.
My main question is that, can levosulpiride withdrawal cause such severe symptoms (especially since I have been taking it for so long)? I am not finding much literature/information about this on the internet, and most of the doctors I ask say that it should not be affecting me, but I am really perplexed as to the cause of my severe anxiety symptoms. Keeping in mind that the dyspepsia, nausea and IBS symptoms have actually increased, I am wondering if the anxiety could also be a result of this?

Brief Answer:
yes levosulpride withdrawal can cause such symptoms theoretically

Detailed Answer:
Hello XXXX,

Thanks for contacting me directly. Sorry for somewhat late response.

I just checked our past communication.

Yes levosulpride after taking for 8 years can cause withdrawal symptoms which you are feelings. As well as IBS and anxiety disorder itself can present with all these symptoms. We cannot say exactly which factor is playing role in which extent but all are there.

The best medicines from your history I guess would-be Escitalopram and/or duloxetine with etizolam kind non sedative anti anxiety medicines.
Bisoprolol I do not recommend. Sertraline is also not recommended as it causes some gastric side effects.

You can discuss these with your treating doctor and change the treatment.

Hope I have answered your query, I will be happy to help further.
Regards,
Dr.Chintan Solanki.
