Suggest Treatment For Seizures While Suffering From Cough And Cold

Question: Hello Doctor,
my son is suffering from epilepsy since 2011. He got first 25/12/2011,second 25/04/2012,third 19/07/2015. Treatment was started after second one.Till third he was taking valparin chrono 500mg+500mg. In the month of feb he suffer with cough & cold at thta time he was given piriton syrup now 15/02/2015 again get seizure in evening 5 pm & again in morning 8 am 16/02/2015. Now he is taking levepsy 250mg 2+1+2 with valparin 500+1000mg but this time he is not well partial seizures are still dailly. All mri,eeg &ct scan are normal suggest me good for him

I am Dr.Ajay Panwar,a neurologist,here to answer your query.

I feel deeply concerned about your son.After going through his medical history,I suggest you to please upload a video of his partial seizures,so that I can see what is the exact semiology of the seizures.

Regarding medicines,I can say that he is not currently on the maximum optimal dose of Levetiracetam and dose can be increased still further well upto 2ooo mg per day.Valproate(valparin) is not the drug of choice in case of refractory partial seizures and I am doubtful,whether increasing its dose still further will help stop the seizures.Rather adverse effects like weight gain,hair loss and tremors may creep in.
Instead,Frisium(clobazam) or oxetol(oxcarbamazepine) are good choices as add-on drugs.Clobazam is specially effective in controlling refractory seizures and Oxetol is one of the first line drugs for partial seizures.

Coming on to investigations part,MRI,CT and EEG coming normal in a case of refractory seizures mandates that the patient should be evaluated with the help of a 'video EEG and MRI Brain with Epilepsy protocol'.

If he continues to have seizures despite being on optimal antiepileptic therapy with multiple drugs,he is a candidate of 'Epilepsy surgery'.'Epilepsy surgery' has a very good success rate in controlling seizures in cases of refractory seizures.

Overall,I would like to say that the place you stay at 'Khatauli' is a small town and not the ideal place to evaluate and manage a case of refractory seizures.
I suggest you to please consult 'Dr.S.Raghwan' at Safdarjung hospital,New XXXXXXX to ensure proper evaluation of your son.If he gets controlled on drug therapy,that's well and good.If his seizures don't get controlled on medicines,please visit Dr.Atma XXXXXXX Bansal at 'Medanta-The Medicity' XXXXXXX for opinion and plan of 'Epilepsy surgery'.
