Suggest Treatment For Schizophrenia

Question: Hi im in need of help . For the past seven years I have been dignosed with scizoprenia. All the medication that I have been taken either makes my symptoms of scizoprenia worse or the side effects are horrible . I need an option to know what to do. When my dr feels the need to change my meds the time period when I don't have meds in my body I feel fine but I have to wait for it to tapper off and that is the worse.

You are a 31 years old adult female and have been diagnosed with schizophrenia since last 7 years. Commonly used medicines resulted in side effects and the medicines either made side effects or symptoms worse. You have been diagnosed with pro cancerous lesion in vaginal cancer and been successfully operated. You are currently on Haloperidol injectable form.

Haloperidol is a first generation antipsychotic (FGA) drug. The drug is known to produce side effects like Extrapyramidal symptoms, though risk in minimal in injectable form.

You haven't mentioned that which other medicines you have taken. Please mention in follow up question so that I can help you in proper management.

Second generation anti-psychotic drugs are relatively safe and are free of serious side effects. These drugs provide effective symptoms control without affecting day to day functioning. As you have taken injectable forms so you have to wait for some days for drug to be cleared from body.

Please provide me details of other medicines if possible.

Hello thanks for providing details,

Haloperidol, Prephenazine both are first generation antipsychotic drugs and Risperidone consta, Olanzapine (Zyprexa), Ability (Aripiprazole) are second generation anti psychotic drugs.

As you have been on both generation drugs and had side effects in such case newer drugs can prove helpful for you. Medicines like Asenapine can be used. The medicine is available in oral forms and it can help to control symptoms of schizophrenia without causing serious side effects. The drug is relatively safe to use. Ziprasidone, Qutiapine are other options which are relatively safer.

Secondly if you are having side effects with injectable haloperidol then along with this drug medicines like Tryhexiphenidyl (THP) or Propranolol can be used. These drugs will reduce the side effects of drug and will result in effective symptom control. You can discuss with your psychiatrist for prescription of these drugs.

Just to sum up-

- Instead of Haloperidol other drugs like Asenapin oral can be used. The drug is newer drug and relatively safer. Qutiapine is other option.

- Or along with Haldol drugs that prevent side effects like Propranolol or THP can be used. These drugs will make haldol tolerable.
