All patients with signs and symptoms of RLS should be tested for iron deficiency and corrected promptly. Complete iron panel should ideally be advised as ferritin alone can be raised as its an acute phase reactant and may mask an iron deficiency.
Secondary causes of RLS can be considered if there are abnormal findings on neurological examination or there is poor response to drugs.
The following should be obtained in that scenario
CBC
Renal function tests (BUN, creatinine)
Electrolytes
Thyroid function tests
B 12 and folate levels
Needle electromyography ,nerve conduction studies (with polyneuropathies or radiculopathies are suspected) and Polysomnography (especially with sleep disturbance) can be advised additionally if the above are also normal.
Benzodiazepines or anticonvulsants can be of additive effect and can be tried in combination with the pramipexole which you are already using.
