Question: hello sir i am dr XXXX i have taken fluoxetine 80 mg for refractory ocd for about six years now obsessions are 90 percent controlled so i am not currently taking it now but sometimes i have mood fluctuations also so currently i have started aripiprazole 20 mg now problem is that i feel sleepy in the day my friends say that i am not alert i am preparing for pre pg exam what should i do now can i take modafinil to increase my alertness and cognition i want to increase my perfomence also help me thanks in advance

Important aspects of your query are: • 28 year old medico • Known case of OCD • History of good response to Fluoxetine 80mg • Complaints of: Mood fluctuation and somnolence with aripiprazole. Aripiprazole is an antipsychotic medicine. From your history reason of its use is not clear (mood fluctuation is a vague symptom and can occur in anxiety, depression and psychosis). Somnolence may be side effects of aripiprazole in your case. If it is started by psychiatrist in your case, then there is no problem as psychotic symptoms can occur in any person with past history of OCD. But if you have started it by yourself or some non psychiatrist friend advice, then you need opinion from psychiatrist. As you are medico and preparing for PG then you should be great mental help to give best in exam. I would recommend you to meet me in Gurgaon or some nearby good psychiatrist for assessment and advice. Bharatpur is only 200km far from Gurgaon. My root origin is also from nearby Fatehpur Sikri and I already have many patient from Bharatpur. sir i will meet u later defenitely but now i need ur immediate advice as i am worried my pg preparation i am posted in govt hospital in chittorgarh and also preparing so need ur help arip mt 20 was prescribed to me by a psychiatrist now i feel sleepy can i use modalert 100 mg?????// or any other drug regards dr XXXX

Brief Answer: Hi Detailed Answer: Hello again, Thanks for follow up. I can understand your concern, but same time it my responsibility to avail you best treatment. You had history of OCD and now you are on aripiprazole 20mg for mood fluctuation (which can occur in ocd, anxiety, depression and psychosis). There is need to understand your case by in person evaluation before changing or starting new medication. You live far and preparing for PG, but it is not more important than psychological health. Even better mental health will give you more strength to clear pg exam, and I had witnessed this by treating junior doctors. You can take leave, if cant, then you should meet nearby psychiatrist. Meantime you can start modalert 100mg after informing your current psychiatrist to overcome sleepiness.
