Thanks for your question on HCM.

Pseudomonas is a gram negative bacteria and usually colonize diseased lungs. So there must be a underlying lung disease like COPD or bronchiectesis or cystic fibrosis or bullous lung disease.

The best way to treat recurrent pseudomonas infection is by giving sensitive antibiotics.
So get done sputum culture and sensitivity report.

If sputum is not available then get done bronchoscopy and XXXXXXX (Bronchoalveolar lavage) fluid culture and sensitivity.

So consult pulmonologist and discuss about these.

I want to know few things about you.
1. Do you smoke?
2. Have you undergone chest x ray?
3. Have you undergone CT thorax?
4. Which antibiotics other than ciprofloxacin you have taken?

Follow up: Dr. Drkaushal85 (12 hours later)
Started around September/October last year.
Given Augmentin for 10 days.
Then given a 2nd different antibiotic - can't remember which, 2 days into that, had spit test and then that one was cancelled and given the ciprofloxacin for 10 days.
Then as CRP came back up, given azithromycin for 7 days.
Lungs sounded good then, 2 days later lung pain (left) has returned.
Blood tests CRP high, and anemic.
No other tests done as yet.

3. Have you consulted pulmonologist?
4. Sputum culture and sensitivity done or not?
Irrational antibiotics should be avoided in pseudomonas infection.
We need to give antibiotics which are sensitive in culture and sensitivity report.
So pulmonologist's guidance is must.

No

Yes

No, yu get referred by GP here. He hasn't yet, Keeps talking about it.
4. Sputum culture and sensitivity done or not?
Culture I think, sensitivity, no-one mentioned this.
Irrational antibiotics should be avoided in pseudomonas infection.
No kidding, I keep hinting about it, he is confident the cipro killed it and goes off on a tangent - lung cancer etc, or some cancer of something else and such.
There are no symptoms other than lung pain (comes back) and wheezy.
And the anemia etc...but my husband doesn't eat much...maybe 1 toast in morning and a preschooler size meal in afternoon.
weight was 61kg, now 56 - he has always had poor sense of smell, now it's worse - in last year or so. So he doesn't eat.
Doc dismissed that bit of info too and thinks weight loss is a symptom.

In my opinion you should immediately consult pulmonologist. You should demand for the same.
As your husband has weight loss too.
So no point in waiting and changing antibiotics.
Rather to diagnose him first and start antibiotics according to the sensitivity pattern.
Chest x ray is must for him.
So demand for pulmonologist consultation as soon as soon as possible.

I can understand your point. Weight loss due to poor oral intake, I know this. But the reason for poor intake is mostly infection.

Pseudomonas infection can cause decrease in appetite and poor oral intake.
So my point was, he is a candidate for pulmonologist consultation because he is having weight loss and this can be due to pseudomonas infection. As pseudomonas can cause decrease in oral intake.
So, weight loss in his case is supportive evidence that he needs proper diagnosis and treatment.

XXXXXXX he will tell you himself.
BEFORE he had the germ and was fine, he went off eating because nothing has any taste anymore....he never ate much anyway.....often when out working he just skips lunch.
Breakfast is usually 1 toast or 1 banana, then nothiing till dinner.
Dinner - 60grams meat, 1 golf ball sized potato, 3 tablespoons veges.

In my opinion his diet is not proper.
It is low in calories as well as low in Nutritive value too.
So this might lead to a weakend immunity.
And low immunity can favor infection more.
So better to treat infection first.
