Brief Answer:
It should be drained.

Detailed Answer:
Hello!

Thank you for the query.

This needs to be drained. It seems that there is some pus collection in the scab area. And as the inflammation is spreading, swab test should be done and antibiotic prescribed. She should also hold this hand elevated especially when lying down.
Please take her to general surgeon or dermatologist.

Hope this will help. Feel free to ask further questions.
Regards.

Brief Answer:
It is not contagious.

Detailed Answer:
It can be due to bite or some mild injury. This looks infected by a bacteria. It is not contagious but can get worse if left alone. Advil wont cure it. Antibiotic is necessary.
