Suggest Treatment For Premature Ejaculation Along With Chronic Back Pain

Question: Hi
I am having chronic back pain for 4 years. Off late I am seeing that I am having premature ejaculation. I am getting erection, but after it becomes hard, i get the urge to ejaculate in 1 or 2 strokes. Can this be because of my chronic back pain. Does chronic pain cause any weakness to my nerves because of which this is happening? I have this premature ejaculation problem for the past 3 or 4 months. Earlier it was fine.

I have read your query and understood your concerns.

I have gone through your previous query also and came to know that this back pain interferes with your work and makes you worried. I want to ask you that dose this pain makes you depressed? Do you feel sad all the time?

Anxiety and depression because of particular stress can cause premature ejaculation. Once premature ejaculation has started, performance anxiety makes it a big problem every time you go for sex. This makes you more depressed and your condition worsens. This continues as vicious cycle.

I know that your back pain is present for last 3 years and sexual problem has started just before 3 to 4 months, so I want to ask you that, is there something else that is bothering you like financial issues, conflict with partner, work load at job, death of loved one etc?

In my opinion premature ejaculation is not because of the back pain but it may be because of stress and depression arising from back pain. You have mentioned in your previous query to other doctor that your MRI was normal, so it is clear that you do not have any abnormality in vertebral body or disc. So nerve compression is not possible. Premature ejaculation is not because of nerve compression.

Here are the tips to manage premature ejaculation:
1. Try to avoid ruminating about having premature ejaculation.
2. Do not let your mind distracted by any kind of stress while sex.
3. Use positions which gives less sensation over penis like woman on top position. This you can identify by yourself also by trying different positions.
4. Learn how to use the stop-start method and squeeze technique. You can learn about these behavioral modification techniques on the internet because it will be lengthy to describe here.
5. You can use extra time condoms which contains benzocaine which is a anesthetic agent which allows you last longer.
6. Spend more time in foreplay before going for penetration.

I usually do not prefer to give drugs like SSRI antidepressants which can delay ejaculation as a side effect. I usually ask my patients to learn the techniques available other than drugs to delay ejaculation because once a person starts using a drugs for this purpose he becomes dependent on it and obviously I don't want to make my patients dependent on drugs for sexual pleasure.

If you are feeling depressed then take a visit to psychiatrist. He will manage depression as well as premature ejaculation. Even antidepressants are indicated in chronic pain conditions.
