default
Posted on Fri, 7 Aug 2015
Question: I have pitting edema in my ankles and calve and feet. I have been checked via chest xray and labs for CHF. All clear. The only thing that was a concern is the AST and ALT levels are both HIGH. I do have RA and take Humira and Leflunomide. I take 135 mg Trilipix for high Trigs. A1 C was normal, Fasting Blood Sugar was 81. Doctors say its the RA. But one of the RA doctors on here said, no it sounds like vein insufficiency. Here's what's going on: I'm a remote field medic I 'v been at work since XXXXXXX 16 th I go home July 31. I need to know what specialist I should try to get in with? My PCP is a joke trying to find a new one. I live in XXXXXXX Alaska.
My legs are itchy , I've tried cream . They are still pitting. woke up with tingling in toes. Any and all advice is greatly appreciated. Also if you know what this is or what causes this greatly appreciated.

Brief Answer:
You might be having possible side effect to Trilipix, Leflunomide or Humira

Detailed Answer:
Hi XXXX,
Thanks for writing in to us.

I have read through your query in detail.
Please find my observations below.

1. There is a possibility that Trilipix, Leflunomide or Humaira might be responsible for causing swelling of legs and the increased AST and ALT levels.

2. The above is a well known adverse effect of Trilipix, Leflunomide and Humaira. I would like to know if the leg swelling and high AST and ALT levels were more noticeable after taking a particular medicine from among Trilipix Leflunomide and Humaira.

3. You might consider discussing the relation between Trilipix, Leflunomide and Humaira and your leg swelling and abnormal AST and ALT with your doctor and if any treatment changes are required.

4. I suggest you discuss the above with a cardiologist and then a rheumatologist. Venous insufficiency is the next probable cause in the list.

5. There have been many patients taking medicines similar to Trilipix, Leflunomide and Humaira and have problems with swollen legs which might require dose adjustment or change of medicine.

Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Hope your query is answered.
Do write back if you have any doubts.

Regards,
Dr.Vivek
