Suggest Treatment For Persistent Dizziness
Question: Hello, my daughter is 13 years old and has been experiencing dizziness since November of 2016. She had an MRI (normal/negative) and saw the ENT doctor, tests normal were normal, but said her symptoms point to vestibular migraines. She has missed months of school because the dizziness seems to be more pronounced in the morning.
We received Topamax for the migraines and Meclizine for the dizziness. She's finally reached the full dosage of Topamax (100mg) on March 28, this past Tuesday. She said the migraines seem to have subsided somewhat, but the dizziness persist.
The neurologist suggested making an appointment with the cardiologist also because her blood pressure has been low, although I though it may be low due to the medication. She is also on 20mg of Prozac. Do you think she just may need to try a different medication or could it be something else?

I have gone carefully through your query. The diagnosis and treatment seem to be proceeding in the right track. However, discuss with her doctor whether they think vestibulocochlear stabilizing agents like cinnarizine or betahistine would help. It might also be advisable to get an audiometry done.
Not sure whether the low blood pressure is an independent factor or whether it is an adverse effect of any medication. A cardiologist would perhaps be able to say better.

I will inquire about cinnarizine and betahistine. Are these medicatoions for dizziness? Is betahistine a beta blocker? If so, she was prescribed Propranolol, but I wanted to wait until she saw the cardiologist before giving it to her because she's already been taking the Topamax for almost 4 weeks now and the neurologist said it would take about 4 weeks for her to get the full effect because of working up to the dosage.
