Question: i was admitted to hospital with syncope and sob 7 days ago due to medication induced orthostatic hypotension. during my bp med adjustments. i was not responding and eventually ended up on 4 bp meds and hydrochlorthiazide. dropped too low to 110/50. came home 3days ago feel slowly better but still experiencing light headed and milder sob. what is the time frame is it normal to take a week- few days ? i was on the cardiac floor of a major teaching hospital in new york and did well with an echo ekg and all blood work over 4 days

Brief Answer:
Bp adjustment problem

Detailed Answer:
Hi,
I am Dr Mittal.
I have read your message.
I think I can help you.
There are 2 possible causes.
1. Your bp is still getting low occasionally. But not low enough to cause you to faint. But enough to cause symptoms.
This will need a further dose adjustment.
2. Probably your bp was very high. Now at 110 by 50 it's normal , but too low compared to previous bp. This is why your brain , previously adjusted to a high BP is not feeling good and causing symptoms. It should come down slowly.
I also suggest testing a CV DOPPLER if not tested already.
I have tried to make it as simple as possible. I hope that the information contained in this message will be able to help you find the best solution for your problem. Best of luck. Dr Mittal
