Suggest Treatment For Nausea

Question: I still can’t get rid of my nausea that comes and goes during the day. 32 yr old male 175lbs. active.



Gasterointeroligst treated me for GERD yesterday with prilosec 2x daily, I have had this for 10 yrs. well controlled but never nausea. Dr. just wanted to treat the GERD and not the nausea.



I am taking claritin 1x and fluticasone propionate 2x daily via allergy doctor. Just started allergy shots last but isn’t helping with nausea. I use netti pot daily. Afrin and sudafed on occasion but no difference. I thought it might be post nasal drip irritating my throat area, but I don’t see a difference.



It comes and go, it makes me anxious when it comes. My throat get kind of tight and I just feel like I want to gag, then it lingers and sits and eventually goes away 1-2 hrs later. Coffee makes it worse, although I can go days without caffeine and still get it. I feel it in my throat, not my stomach.



1. How can I solve my Nausea?



2. Is it possible the fluticasone propionate is giving me nausea and increasing anxiety due to it being a steroid? I am one of those people that is sensitive to everything (season allergies, caffeine, etc.) I was planing on stopping it today to see if it would go away. I understand that it takes some time for that drug to leave the system, like days?

You are having allergic post nasal discharge &GERD

Detailed Answer:
Hi,
Thanks for the query.
I have read your query , reviewed your medications and understand your concern.
As per your clinical history it appears that you are having allergic post nasal discharge & GERD .
If I were your treating doctor, I would advice you
change prilosec (Omeprazole) to Esomeprazole+Domperidone combination ,
claritin to Montelukast + Fexofenadine combination ,
stop fluticasone propionate for 7 days ,
continue allergy shot require long time to see effect ,
continue netti pot daily &
stop using Afrin and sudafed.

Nausea will go by Esomeprazole+Domperidone combination .

Since your are using nasal spray steroid for long time give gap for 7 days. Use warm water gargle after 20 mins of nasal spray use to remove any spray coming in throat.
For GERD life style modification is most important . Avoid coffee , alcohol , smoking , fast food if any & take food 3 hr before sleep .

I would advice you to do Investigation like Diagnostic Nasal endoscopy & CT scan Paranasal Sinus to see status of sinuses & extend of disease in sinuses .

Also you need to do Videolaryngoscopy of your throat & larynx to see present status & any side effect of long term use of nasal spray steroid .

Ok I will make an appointment with an ENT. It sounds like this is their specialty.

Why do u suggest nexium vs. Prilosec? Is there an advantage?

Domperidon is not available/approved in U.S. Where I live. Can you suggest something similar to ask ENT to prescribe?

Yes, I have drastically altered what I eat for GERD and have read up on lifestyle changes extensively.

Great idea about salt water if I ever go back to the nose spray. I did not take nose spray last night or today and gargled salt water but feel just as much nausea. Hopefully it is the nose spray still in my system?

I've had allergies since a teenager and have taken Flonase for years. I've never experienced the nausea until 8 months ago. Does the most just change and not handle post nasal drip and the nose spray out of no where?

Nexium is better for upper GIT problem like GERD , esophagitis than Prilosec .

Use Ranitidine twice daily along with Nexium for nausea .

Continue life style modification for GERD .

Use warm water gargle after 20 mins of nasal spray use to remove any spray coming in throat .

For allergy change claritin to Montelukast + Fexofenadine combination.

You can discuss this with your treating ENT doctor & get the above mention investigation done .
