Suggest Treatment For Moderate Scarring In The Lungs

default
Posted on Wed, 23 Apr 2014
Question: Earlier this week my PCP told me that the X-ray and MRI showed that I have moderate scarring in the lungs. I have had pneumonia 4 times in 2 years. She said there is no cure and I wonder what the outcome may be. My age is 82 and I was put on oxygen therapy in Feb. for asthma (I think). I would like to know the causes. I have Never smoked or for the most part have been around 2nd hand smoke. For the most part I think that I have been healthy until I turned 80. I do have Fibromyalgia. What seems to bother me most is Severe Restless Leg syndrome. For RLS I take Requip and Lyricia and hydrocodone. I still have whole body pain from the RLS & Fibro. I do have some chest pain but my cardiologist says that my heart is in good condition for my age. I do have high blood pressure.

Brief Answer: You need to go through HRCT scan thorax first Detailed Answer: Hello Thanks for your query and I admire your concern. From your history it seems to me that you might have been suffering from some fibro-bronchiectatic changes over lung which may be a consequence of repeated episodes of pneumonia. You need to undergo a high-resolution computed tomography scan (HRCT scan) of thorax to delineate your scarring of lung properly and thoroughly. If it is seen to be extensive then probably no cure is there. In that case all we can do is conservative management like chest physiotherapy, infection control through pulmonary vaccination (pneumococcal vaccination and influenza vaccination), bronchodialator therapy, pulmonary rehabilitation and treatment of complications like pneumonia and hemoptysis which may arise in this case frequently. If on HRCT scan thorax the scarring is seen to be localised involveing smaller area of lung then you may go for lobectomy after consultation with a cardiothoracic surgeon to cure your disease. Therefore you first go through a HRCT scan of thorax and spirometry testing and treatment decision will be taken depending on the result of investigations. Also you should have a proper control of blood pressure through modification of your existing medications. Thank you Regards Dr Arnab Maji MBBS, MD Consultant Pulmonologist
