Suggest Treatment For Malignant Non Hodgkin Lymphoma

default
Posted on Mon, 14 Jul 2014
Question: My Father, XXXXX, has been diagnosed with Lymphoma-Non Hodgkin and the same is suggested as malignant. I and the entire family is extremely disturbed for last one week.
He has been already operated for Lymph Nodes removal and treatment awaited for Chemo-Therapy.
As suggested by XXXXXXX Sir, after discussion with you, I tried to log in the Health Care Magic, but was unable to send the documents to Oncologist for a second Opinion.

I would humbly request you to please help me to get a second opinion on the Treatment as I am extremely disturbed as I am jostling with the Treatment alone.
I have attached all the reports ie Biopsy Report & Pet CT Scan Report for your perusal and due suggestions. If any further details are required, I can personally speak to anyone for the same.

Brief Answer:
Will require chemotherapy

Detailed Answer:
Hi
thanks for your query.
I can understand your concerns. However, I can see only a PET-CT report which has been uploaded. I am still unable to see the biopsy report.
Having said that, it is well known that malignant non Hodgkin lymphoma is a treatable disease and responds very well to chemotherapy and radiotherapy. Hence the treatment plan seems appropriate.

If you can send the other reports including the biopsy and doctor's assessment, I can revert back with more details on the treatment protocols, side effects and long term results. If you are unable to upload them in the system, you can send them by mail to YYYY@YYYY .

Warm regards.

Detailed Answer:
Dear Mr XXXXXXX

It appears that your dad has got a low grade lymphoma (most likely follicular lymphoma). This is based on the PET-CT report and description of the cells as per biopsy report. Final diagnosis will be on immunohistochemistry.

Also It appears to be stage III disease (on a scale of I-IV). If it really turns out to be small cell low grade NHL then treatment is by chemotherapy (along with targeted therapy rituximab if CD20 is positive on IHC). Results are quite good in terms of response and disease free interval. However, low grade NHL in stage III is incurable as on today's date. If it turns out to be high grade, then it may be curable.

In either case I am sure he has many years to live a healthy and active life. So don't get depressed as he needs his family to support him through this phase more than anything else.

Please write back if you have any further queries.
Regards

Dear Sir,

Thanks for your Answer!

Can you please explain this statement" However, low grade NHL in stage III is incurable as on today's date. If it turns out to be high grade, then it may be curable."

Does it means This Low Grade NHL is incurable?

or it means that it dosen't need any Treatment as it in low grade?

Also i will share the IHC Report once i receive it.

Thanks

Detailed Answer:
Dear mr XXXXXXX

Your queries are indeed very relevant. my answers to them...

Q. Can you please explain this statement" However, low grade NHL in stage III is incurable as on today's date. If it turns out to be high grade, then it may be curable." Does it means This Low Grade NHL is incurable?

A. yes, it means that low grade NHL in stages III-IV is incurable. But newer therapies are coming up and in near future it may become curable. Nobody knows!

Q. or it means that it dosen't need any Treatment as it in low grade?

A. yes, some low grade NHL in stage III-IV are not treated if they are asymptomatic. Treatment is started when they become symptomatic or have low platelets or hemoglobin in blood. This is because early treatment in asymptomatic patients do not extend survival.
