To recapitulate: Female/30 - complaints of mid and low abdominal pain, irregular bleeding, low back pain nausea, fatigue - has gained 10 lb in one and half month and negative pregnancy test.

Is there edema/swelling over the legs over the shin/ankle?
Has the face become rounded?
Do you get fatigue more in the evening or is 24/7?
Do you have any associated medical problems/taking any treatment or got any investigations or medical consultation done?

All these symptoms and finding at the age of 30 make me think about hypothyroidism as the first cause in the list of my differential diagnosis.

I would advise you the following to get to the diagnosis?
Tests of blood: Routine and particularly Thyroid Function tests (Free T3, Free T4 and TSH), liver functions, kidney functions, Lipid profile, Blood sugar and as may be advised after clinical evaluation.
Clinical evaluation and examination by a Physician- this is also very important to find the signs and probable clinical diagnosis to be supported by the tests to come to a final diagnosis to get a proper plan of treatment.
