Question: hi I have had lower back and pelvic pain for 2 years I am under the hospital for enlarged prostate but tablets are not working I feel like I want to pee all the time and now I am getting sharp pains every now and again in my pubic area

Any other investigations.

I have had blood test done on prostate all clear had ultrasound on bladder he didn’t say anything was wrong had back passage checked that is when he said I had enlarged prostate I am tired all the time to

Tamsulosin works for Benign Prostatic Hyperplasia (enlargement).
Vesicare is given for another condition called over-active urinary bladder.
Hence You are most probably suffering from Over-active bladder and this is difficult to treat.
I would advise you the following in such a situation:
Take an opinion of a Urologist or take a second opinion of another Urologist if you have already shown to one.
You need further evaluation:
Uroflometry and further investigations like Urethrocystography, Intravenous Pyelography and so on..
MRI of the lower spine to rule out any spinal pathology as you have back ache too.
This may be affecting the bladder as well as causing the pelvic pain either referred or due to bladder condition.
This will give the exact cause and thus help for appropriate management.

I can understand your anxiety as you are suffering for a long time as well as tired all the time.

Take proper further steps which will help further.

I have had my urine measured and results was a poor stream that was first time I seen urologist he said my prostate was enlarged and inflamed was given a course of antibiotics and tamsalosin for three months it improved for two months then went back to normal as I said he is know treating me for overactive bladder been taking tablets for thre weeks it hasn’t improved I am always very light headed too I have had a mri scan on my lower back and it was clear

It is good that the MRI is normal.
Overactive bladder is difficult to treat as the cause is not always known and one needs complete evaluation to know the cause.
The probable causes can be as follows:
Enlarged prostate - as you already had it.
Urinary tract infection.
Possible postvoiding residue that is incomplete emptying of bladder.
Excessive use of caffiene/tea/alcohol or any beverage that causes excessive urination .
Stress and anxiety are well known causes.
Neurological causes.
Diabetes and so on.

Try to find out what instigate or enhance your problems.
Discuss in detail with your treating Doctors/Urologist.

Light headed may be related to your stress or anxiety; try to find out the cause.
Take medications regularly, as per advised and consult/review with your Doctors regularly. It may take time but different sort of medications are now available and help to control overactive bladder.
