Question: Need URGENT ANSWER. My Direct Premium Query to Dr XXXXXXX Heijebu, upon receiving your email. We highly seek your help in your issue.I have persistently getting low sodium levels in the blood. This has been happening since 2 years.I do not have any symptoms.Initially my doctor said ,it is because of hypothyroidism.I am on eltroxin 50 since 1 year.but another doctor said it is not because of this thyroid problem, since I have euthyroid status now and that problem existed even before diagnosing hypothyroidism primary.I do not have BPH.I even stopped taking hydrochlorthiazide 1 year ago ,when doctors said it is due to diuretics. But still it is not resolved.I am totally confused.I have levels of sodium around 115 meq/l.I know this is severe hyponatremia ,but no symptoms.I read about the symptoms of severe hyponatremia, but I do not have them.I am afraid. What could be the reason, I have got all scans -CT, USA,MRI as normal. My kidney tests are also normal. Please guide me, I am fear struck. My epeilepsy scan also normal. My doctors are clueless of my condition, they are finding it surprised and no ideas??????My BP is 130/80 pr 66,ecg with lvh may be due to htn I do not know.Normal bowel ,no excess urine,no bph.my brother has bph i do not have it..Need your urgent answer and guidance.i am deeply troubled. respond at YYYY@YYYY

Brief Answer:
Need further tests.

Detailed Answer:
Hello, Sir.

I understand your concern.

I have gone through your query in detail.

The cause of hyponatremia in your case need further evaluation.

The possibility of hypothyroidism and hydrochlorothiazide related hyponatremia can be reasonably ruled out based on the explanations offered by your doctors which seem prudent and scientific.

Since you are asymptomatic, there is no much need to worry.

But it is very important that sodium levels are therapeutically increased once the cause is found out.

For this, I advise you to get the following tests.

1.Urine and Plasma Osm.
2.24 Hr-Urine electrolytes.

These tests are important in determining therapeutic components.
