Suggest Treatment For High Uric Acid Levels

Question: my wife aged 24; due to kidney stone and infection her right kidney was damaged As it will removed on 6 th of XXXXXXX and find there Diagnosis : Right nephrectomy - Granulomatous Pyelonephritis consistent with Tuberculosis.
so she is started medicine of Tuberculosis from 26 th of XXXXXXX below:
1.cap. Rcinex 450 mg- 1 cap daily
2. Tab combutol 800 mg - 1 tab daily
3. Tab Pyzina 750 mg - 2 tablets daily
4. Tab Benadon 40 - 1/2 tab daily
5 . Tab pantocid dsr - 1 tab daily
but now she is facing pfoblem due to uric acid high level 6.5 and also feeling pain joints of hands and legs deeply like Gout.
so what she needs to take as these drugs are running already 34 days as we need to going on..
so here shall we reduce pyzina dose like shall we give her 1000 mg or else pls tell me the exact right dose for pyzina in this situation ..
Also should she need to take any uric acid reducing medicine ..
Her current weight is 58 and hight is 5.3 inch and bp is 102/67..
so pls check and get back to me asap

Detailed Answer:
Hi Sir,

I understand your concern.

This is a an unusual presentation.

Both Pyzina and Combutal are known to elevate serum uric acid levels.

Elevated Uric acid does not always confer diagnosis of Gout.

Likewise Gout does not always demonstrate elevated levels of uric acid.

Value of 6.2 is on higher side of normal range.But as said earlier ,this does not rule out Gout.

The reduction of dose of Pyzina or Combutal is not advisable,since curative efficacy will be decreased.

I advise your wife to get a 24 urinary Uric acid levels instead of Serum Uric acid levels.

Introduction of medicines to reduce the production of uric acid is wise in your case.

Drugs like Allopurinol(reduces uric acid production) has to be instituted ,since a diseased kidney will not be able to excrete uric acid at a normal pace.

The dose of allopurinol can be accurately determined after testing CrCL(creatinine clearance of kidney).

Usual safe dose is 100 mg PO one a day for 3 weeks.

Consult your doctor with reference to above.
