Question: Hi Doctor,

Please help us....!!

I XXXXXX from India - Bangalore got your information through net from your website. Doctor my mother suffered heart attack on 7th aprill around 1:15 AM and the code blue team indicated us that to get her pulses back it took 20 minutes. Doctor the history is My mother is diabetic, bp and her angiogram for heart was done during 2008 in which she was on continues medication and she use to take insulin for 3 times a day....

However, on 10th march we admitted her to Manipal jayanagar in blore for cough and high fever, she was in hospital till 29th march 2014, then we got discharge, but she was on medication as well for the same as doc stated that she has phenmonia in right lung.... again on 4th april 2014 her cough and fever become 102 degree with sugar levels of 542..... we got her admitted to same hospital and on 7th april in hospital ward she got this attack and now she is in coma since 18 days.... there were some movements since 5 days back, but the doctor said... those movements are involuntary not voluntary.... here doctors are telling she might recover or may not for life long... we are in delima... would you please check the attached clinical summary, MIR, EEG reports and help us on this doctor...

Firstly, I am very sorry to hear about your Mother. Her illness is severe and I am sure you and the rest of the family are caught in many dilemmas.

I have gone over all the tests you uploaded in detail. From what I have been able to put together, it looks like during the time her heart had stopped, the Brain was not able to get enough blood flow to keep itself alive and undamaged.

The EEG and CT scan are both showing changes which point to severe brain damage. Now, how much she can recover from this is debatable given her age and her other medical issues.

In the best of circumstances she may regain limited consciousness and open her eyes within a few weeks. If her other systems do not malfunction in the meantime that may happen. The chances of this outcome though are less than 50%.

The more likely outcome is that she stays in the vegetative state indefinitely and her heart and kidneys will fail. I wish I had better news, but from the information you have given here, these are the outcomes I see happening.

Hi Doctor,

Thanks for the information... however today we have received the clinical summary from hospital ...

In view of prolonged ventilation, she underwent tracheostomy on 17th april. REpeat EEG also showed epileptic spikes for which antiepileptic doses were readjusted. At present she is on C-PAP Mode, occasionally open eyes spontaneously, shows cough reflex to suction. Poor neurological prognosis has been explained to the attenders...

Current medication:
Inj Pantoprazole 40mg IV OD
Inj. Clexane 40 mg S/C OD
Inj. Sodium Valproate 1 gm IV BD
Inj. Levipil 1GM IV BD
Tab. Amlodipine 5mg NG BD

Not Brain dead, but bad condition

I have seen the summary and thank you for the update, as i said, she is not brain dead. But the extend of brain injury is severe.

Given her age. I wuld not expect a positive outcome here. All we can really do is prolong her life.
