Suggest Treatment For Generalized Anxiety Disorder With Panic Disorder

Question: I am a 19 year old female. I don't have any current health problems except for panic disorder. I've been having problems with shortness of breath, rapid heartbeat, always feeling hungry, always feeling hot, and always feeling tired. The shortness of breath has gotten worse within the past few months and is just getting worse. Walking up stairs, any from of exercise, or just doing very small tasks make me feel out of breath. I used to go running and could run a 5k without stopping, but now I can't even last 2 minutes. I am also always thirsty. I didn't know if that is because of the being out of breath and breathing more or anything. I'm not sure how long I have had the rapid heartbeat, but it just comes randomly when I am not doing anything. The being hungry doesn't really bother me I just find it weird. No matter how much I eat I am still hungry. Even after eating a big meal I feel starving within a few minutes. I never eat again though. I easily feel hot a lot and get sweaty really easily. That has gotten worse within the past couple months. The being tired makes it hard to want to do anything. No matter how many hours of sleep i get I always feel tired. I never used to take naps, but then last year I started to take a nap when I came home from school. Then it turned into me sleeping in more. Then whenever I woke up I wondered when I would get to take a nap. Now I sleep in till 10 or 11 even though I go to bed at 11 and take 1 or 2 naps every day. I tried not sleeping so late or napping but it didn't make a difference. I always feel dead tired and my eyes are heavy. I don't have a problem with sleeping really. It takes me awhile to fall asleep at first because I always have a problem keeping my legs still, but then I am fine for the night. I don't know if this is related either but just this month I have gained 5 pounds. I am not eating any different than usual and for awhile I was doing my best to go walking with my mom every day and still do some exercise.

Brief Answer:
You need antidepressant therapy

Detailed Answer:
Hi
Thanks for using healthcare magic. I, Dr Abhishek Kapoor, would help you in this query.

I think, you have generalized anxiety disorder with panic disorder. In that case, you should try some antidepressant like paroxetine or escitalopram with low dose benzodiazepine. You can also try some relaxation exercise like JPMR or deep breathing. It would keep you calm and relax. Better to consult a psychiatrist for proper management.

Weight gain in your case could be due to stress or anxiety. In anxiety or stress, level of corticosteroid hormones increase and it could lead to weight gain. Nothing to worry about that. It would get controlled by medicine.

In case, you have further query, you can ask.

I am already on a antidepressant for my panic disorder. I have been on it for over a year. My panic disorder is a lot better now. I rarely get any of the attacks I had and my anxiety levels have gone down a lot too. I haven't had problems lately with with feeling anxious, nervous, or stressed really either. And I have also been in counseling for awhile to help with my panic disorder and the counselor helped with any anxiety or stress issues and how to manage them, and that hasn't been a problem for me anymore. Is there something else it could be?

Brief Answer:
Get thyroid profile done

Detailed Answer:
Hi

At your age with such kind of symptoms, i could keep few possibility like hypothyroidism, cushing syndrome and anemia. Tiredness and weight could be possible in all of these disorder. You can also get your hemoglobin, thyroid profile, hormonal assay, liver function test etc done. That would help to find out the root cause and plan further management.

In case, you need further help, you can ask.

Can hypothyroidism be genetic? I know that my mom has some sort of thyroid issue that she takes medicine for every day. When I go to see my doctor how should I approach the symptoms I have been having? Should I just list every thing like I did to you and let her decide or should I also suggest getting the test(s) done that you suggested?

Brief Answer:
Inform your doctor about the symptoms

Detailed Answer:
Hi

Just tell your doctor the symptoms and about family history of thyroid in your mother. If any one in family has any endocrinological disorder, then chances of getting the same increases up to 20%. In that case, your doctor would suggest you thyroid profile.

In case, you have more query, you can ask.
