Suggest Treatment For Dukes C1 Colon Cancer
Question: My dad has dukes c1 colon cancer . 28 nodes were removed with 10 affected .i need assistance with curability

Sorry to know your Dad is suffering from Colon cancer.
Colon Cancer has far better prognosis than the cancer of other parts of the body. It is a good thing that 28 nodes along with the colon have been removed and only 10 were affected. These must have been in the vicinity of the cancer.
The first step is surgical removal of the cancer and nodes, C1 means the nearby organs are not affected and has a better survival rate than the C2 and 3 .
I would suggest to start the Chemotherapy at the reputed center at the earliest. Oncologist will take over the case and is going to be the mainstay of the treatment. Please follow the Oncologist's adjuvant chemotherapy to have a possibility of cure.
In short; based on the details provided, I see his chances of making through this cancer is high. Regular follow-up is the key to success as the intermittent decisions can be taken towards cure an therapy as necessary from time to time can be instituted.

Ya one thing bit relieving is Colon cancer better than other parts .
What worries me is that is 10 nodes affected a lot?
Yes the nodes were only In the vicinity of the cancer.
What worries me is that can mets appear ? And if so , how soon ?

Since the nodes are in the vicinity and there was no local spread and 28 nodes were removed - only 10 were positive all suggest for a good prognosis.
Chemotherapy by the Oncologist will help to cure the secondaries , if at all by good chemotherapy.
Mets may not appear.

Thinking mets . I know I should not . Last ct scans were feb.
Should be request further ct scan or even pet scan to be on safe side ?
Can he be a c1 though with a t3 tumor and 10 nodes ? I thought that wud be a c2
If he rejected chemo - would mets definitely appear or does it depend ?

All the survival rates of C1-/ 2 /3 are suggestive of the prognosis- I mean they are just suggestive and What is more important to know is that he is a known case of Cancer colon with operated with 10 positive nodes.the nodes are connected to each other by lymphatic channels ans so is the chance which can NOT be predicted and I think should not be predicted.
It depends whether the mets would appear or not if he refuses chemo.
Have a regular check-up and insist on chemo as it helps in Ca Colon with mets.
It would not be worth taking a chance.

Ok but prob is its over two weeks since surgery and he cannot eat hardly . He's lost 1 stone or even more n therefore gone very weak .
Is this normal?

I can get that it is over 2 weeks of surgery .
I think he can have a absolutely normal diet if there is no abdominal problem. The intestines heal very fast and if there is no obstruction patient should be given everything he wants.
Unfortunately at many centers the practice of following dietitian goes on.
This is as simple as - allowing him to eat and drink whatever he feels like. This is really important. Try to understand a patient who has undergone such an extensive surgery ( and might be he knows the diagnosis), his psychology, worries and all.
The best way is to take everything as if nothing has happened and everything is normal approach. This improves patient's self-confidence and better outcome.
I hope this approach will dramatically help him to overcome eating very less. Give him a very normal diet which he used have before surgery.

Hi we are more less giving him what he feels like but he don't feel like eating and gets full he says . Will this pass .
They have checked there's no obstruction but is fluid in stomach .
Will he be fit for chemo
In 5 weeks if not eating and has lost weight ?

This is really good that you are giving him what he wishes for.
There is a difference between ''appetite'' and ''feeling like eating''. If his appetite is good this is a good sign.
Feeling of eating is more affected by Psychology, changed bowel habits due to resection( Surgery), the medicines, its smell and all. And he is saying he is getting full. I hope he is ambulatory (moving around).
Is he passing his stool normally?
We do recommend Tablet Domperidone 10 mg one tablet 8 hourly to enhance the gastric emptying. Please give it only after consultation with your Doctor.
Try some appetizer soups.
IT is good that there is no obstruction.
There will no problem with chemotherapy with reduced weight, he will be ready.
