Please find my observations below.

The kidney has a collecting system where the generated urine first gets collected and then passes in to the ureters which are tubes collecting the kidney and urinary bladder. The pelvicalyceal system is another name for the collecting system in the kidney. A dilatation of pelvicalyceal system in kidney might mean any of the following situations.

(i) It is possible that stones have been found, therefore causing obstruction to passage of urine.

(i) It can be that the bladder was completely full at the of ultrasound scan however if it is involving only one side then this idea is not much acceptable

(ii) The kidney pelvicalyceal system is appearing full by itself (extrarenal pelvis) and is an extension of normal appearance.

(iii) Rarely an infection of urinary system and kidneys (recent or old) can cause such a finding but this is likely only if the patient has symptoms like pain in the region of kidney and problems like burning urination or discoloration of urine or any kidney infections in the past.

(iv) Stones in lower ureter can also cause fullness of pelvicalyceal system and this needs to be cross checked by seeing the ureter opening in the urinary bladder.

(v) Any mass in abdomen including retroperitoneal area causing pressure on ureter.

Consequences of a dilated pelvicalyceal system varies with the cause. If there is a large stone or kidney infection causing it then it can cause kidney damage if not treated on time.

If it is a variation of normal like a extra renal pelvis then it will not cause any problem and requires no treatment.

Should there be any mass causing obstruction to flow of urine is also to be treated.
