Brief Answer:
Detail Follows.

Detailed Answer:
Dear Sir.
Welcome back.
It seems you have developed diabetic/hypertensive nephropathy.
In such situations an important investigation is Ultrasound of Kidney and Urinary Bladder.
Discuss with your doctor about starting you on ACE inhibitors like Ramipril.
Also you have to take low protein diet.
As far as your query is concerned I must say its not perfectly reversible but can be controlled.
Get an Ultrasound done first of all and get back to me.
Stay Healthy.
