Suggest Treatment For Depression And Low Confidence

Question: My brother is Computer engineer. He used to a good student and wants to be a software engineer in big MNC but after completing his BTech , unfortunately he was not able to get any job. He started feeling low in confidence.He stopped meeting with other people and used to feel low all the time. Parents took him to few docs and he was on medications for last few months. During this time he developed a strange issue. His eyeballs start moving left to right and he is not able to maintain an eye contact with anyone. Once eyes start moving he switches all the lights of house and out a cloth on his eyes and lays down on the bed.He starts having headache also during this time. He feels like his voice is echoing in mind and his legs are shivering.



This happens to him in every two three days. I think the heavy medicines are making his mind dead. But don't have dare to stop them. Doctors please help as I cant see my little brother loosing in front of my eyes.

I am attaching the reports and prescription of my brother. Please let me know if he is taking correct medicine,what is his illness, and most important how can I get him back as a normal person. Also let me know the best doctor in Delhi/NCR region whom I can consult in person if required.

His issue began 2 years back. Initially the symptoms were sleepless nights and used to walk around in home thinking and thinking for 2-3 hours in one go.... Now over the course of medication he has developed rigrous eye ball movementand other symptoms as described above..

I think, your brother has depression. Low confidence, not interacting other people, low mood, sleep disturbance, headache etc are symptoms of depression and in that case, he needs antidepressant. He is already on antidepressant like escitalopram or sertraline, but on sub-optimum dose. Dose of sertraline could be increased up to 200 mg per day, but in last prescription on 23rd May, he was on 50mg of sertraline. Along with antidepressant, he was on antipsychotic. This could be the reason of rolling of eye ball or movement in eye ball. It could be the side effects due to antipsychotic like risperidone. If he does not have any psychotic symptoms in form of hallucination, delusion etc, then there is no need to give him risperidone. These drugs would further aggravate the eye symptoms.

If i was his treatting doctor, i would have preferred sertraline with tab aripiprazole or combination of antidepressants. These would help him in improving in depressive symptoms.

Please tell me the exact location in NCR, where you stay, so i could recommend a psychiatrist to you.

Thanks again Doctor,please let me know the XXXXXXX hospital's location. As patient is currently in XXXXXXX it will take time to come here in XXXXXXX One thing I want to confirm is he taking right medicine as these things are looking like side effects of medicines. And how much more time it will take to make him normal. As he is loosing very critical time of his life when he should focus on his career.

XXXXXXX Hospital is in Sector 14, XXXXXXX He is on low dose of antidepressant and that has to be increased. There is no need of psychotic if he does not have any psychotic symptoms.
