Suggest Treatment For Day Time Sleeping And Aggravation In A Person With Drug Induced Psychosis
yes you are going on right track
Detailed Answer:
Hello XXXXXXX
Thanks for writing to us. I can understand your concern for your son.
You have very well narrated history of your son with each minute details.
It seems that he has been managed by a well efficient psychiatrist.
His diagnosis is also seems right 'Drug induced psychosis'.With only 5 mg olanzapine he has improved that is good thing. Cannabis induced psychosis in general takes 1 to 6 months for complete improvement.
His sleeping problem may be due to olanzapine also.Sometime after psychosis some depressive features also develop which we need to address.Changing medicine to aripirazole or some antidepressant may help him more. Proper evaluation is needed to decide which medicine should be given now.His psychiatrist is doing absolutely right thing.
Simultaneously he should not take any substance again that should be taken care with help of motivational counseling and observation.
Hope I have answered your query, I will be happy to help further.
Regards,
Dr.Chintan Solanki.
you should treat him as your son not patient, no pampering, just caring
Detailed Answer:
Hi,
Thanks for follow up.
To counsel you about parenting on this issue we need lots of discussion and which is practically nor feasible on this forum. Still I will try my best to answer your concern.
Though he has some psychological problem, it is not like that he is not aware of reality. You should try to motivate him for study and waking up in very polite manner. None of the parents should be angry whatever the situation. You are right that sometime children learn this type of behavior and take advantage of the same if they are able to get their wishes. If he is doing any wrong thing tell him like a mom should tell to her son. Be gentle in showing his mistakes and wrong manners.Did not accept wrong things or manners due to fear of his irritability.Try to convince him best that whatever he is doing is not going to help him in future.
Detail interview by a psychiatrist needed to know that his irritability/tantrum is part of illness or behavioral problem. If it is part of illness it will be treated with medicines. Otherwise you need to understand approach of positive reinforcement and negative reinforcement to improve his behavior with help of his treating psychiatrist.
Feel free to ask if you have other query.
Decreasing dose will not bring psychotic features
Detailed Answer:
HI XXXXXXX
Thanks for follow up and sorry for delayed response.
I read your query and understood your concern.
His diagnosis is Drug induced Psychotic symptoms. In almost 70 -90% of such psychotic cases there is no relapse if that substance/drug (cannabis) is not taken again.Decreasing dose of olanzapine will not bring back the psychotic symptoms.Rather it may help in sleep aspect.
His delusion if still present it is treatable with medicine. You cannot counsel by yourself. It requires a professional.
However in my opinion fear of forcing by friends for drugs is not at delusion level it is at idea level. He can be treated with counseling and behaviour therapy for the same. It requires motivation to go again to university and challenge his fear.
You being a parent just need to be watchful about his symptoms and behavior and be caring and motivating in positive manner without accepting his wrong demands. If you notice any symptoms or behavior whcih is unusual or related to past episode consult/inform his psychiatrist soon.
Hope this answers your concern.
medicines should be continued for at least 3 months after improvement
Detailed Answer:
Hello XXXXXXX
Yes you are right that twenties is age of onset of schizophrenia. But in case of your son it is clear that present diagnosis is Drug induced psychosis. He is improved that is an important part. If his symptoms continue for at least 6 months without history of use of drugs(like cannabis) we can think about schizophrenia.
This psychotic episode suggests that he is at risk of schizophrenia double than the general population. The first and most important approach to prevent schizophrenia is avoidance of use of drugs. He should be counselled about this aspect by his psychiatrist.
To keep him away from drugs he should regularly visit psychiatrist(even if he is normal) until he completes his study or settled in occupation. As well as being parents you should be watchful with gentle care without being suspicious.
Other aspect is prevention of excess stress in his life. Any type of social, study, occupational, economical or emotional stress should not go beyond some level. He should inform you as well as psychiatrist if he feels such stress.
In my opinion when he starts regular study/work and none of the abnormal behavior or symptoms is there we can say he is completly improved. And after this complete improvement for 3 months we can stop the treatment.
Hope I have answered your query. If still any query there, I will be happy to answer. If not, you can close the discussion and if feasible give your feedback so I can improve service. In future you can contact me directly on this site if you wish with this link: http://bit.ly/drchintansolanki
