Question: My son-in-law, age 40 has been diagnosed with bi-polar and narcissistic personality disorder. He has systematically cut off our daughter and grandchildren from most everyone in order to hide his abuse of them. About a year ago, he started showing signs of paranoia. Then, within the last month, he has been forgetting things, mismeasured a door to be installed, is irritated, and then he left the house in the middle of the night and walked nine miles according to his iWatch. And he has no memory of doing that. I know it is very difficult to help me with such limited knowledge. I am very concerned about his behavior and the safety and well being of our daughter and the grandchildren. (Boy age 13 and girl age 10) I have degrees in communication, counseling and psychology and have been under the guidance of a psychologist who has confirmed everything above, including sociopathic behaviors. I just learned of the behavior over the last three-four weeks that I described above. Can you help me? I can't see Dr. Baker until next week.

I have gone through the provided details and I feel the episode of wandering away with no memory could be dissociative episode or fugue. I must acknowledge that it could be simple manipulation to avoid punitive actions as manipulative behavior is one off the core features of narcissistic personality disorder.

If you are sure that he is not manipulating than I would call it a dissociative fugue and this can be helped by psychotherapy and there is little role of medications particularly benzodiazepines.
