Suggest Treatment For Chronic Migraine

Question: Hello Dr,

My mom is has the migraine issue for almost 10 years. We consulted with few of the experts and every one suggest only to keep away from the mental pressure.

Every time she got the headache, she will also have vomiting and when we take her to hospital, Dr will inject a pain killer and she will be ok for few days. After some days she will get it again.

Kindly let me know more on this about the treatment and other process.

I have noted your mother's history and agree that she is suffering from chronic migraine with frequent headaches.

Giving pain killer injection is not the long term solution, as it would not prevent the headaches from occurring in the future.

The best treatment option for her is to start preventive medications, in order to reduce the severity and frequency of headaches. The good medications for this purpose are topiramate, flunarizine and beta blockers.

In addition, she can take rizatriptan tablet, as and when she gets headaches.

Can you put more clear on these tablets.

When to take and how long she should take and what are the side effects on taking this etc..

1. topiramate
2. flunarizine
3. beta blockers
4. rizatriptan tablet

Thank you for getting back.

Topiramate can be taken once daily after breakfast. It is usually started at a dose of 25 mg once daily and can be increased every two weeks by 25 mg (to a maximum of 200 mg per day), depending on the response.

Flunarizine and beta blockers are needed, only if topiramate fails to prevent headaches.

Topiramate may cause mild weight loss of 1-2 kg and cause tingling of feet. In very rare cases, it may cause kidney stones (less than 1% of patients), otherwise, it is very safe.

Rizatriptan is used at a dose of 10 mg as and when there is headache (maximum two tablets per day, and upto four tablets per week). It is safe and has no side effects.
