Suggest Treatment For Acute Knee Pain After An Injury

Question: halo.. how are you? Dr.herrick. My name is Ferdian. I would like to enquire about my wife current condition. sometime ago, my wife has accidentally twisted her foot. not being careful when she was walking. so as a result she injured her knee. i think her knee has become inflammed so i took her to take a MR image. the result has shown that - Non abnormal sign of bone shape around the joint of right knee, the bottom bone shape interface that support upper bone still in acceptable shape. However there is a damage of level II at the back bottom bone shape which needed to be highlight. Also There is water that appear around upper knee bone that still did not absorb by the body. Soft tissue around the joint of right knee are normal. - It's been a while already.at first i thought it is just normal but as time goes by her knee condition becomes worse so then i had took her to see different specialist.was given herbal medicines,acupuncture and etc. But right now she is still experiencing pain, difficulty in walking (mobilisation). i am worried as to what happened to her knee.what kind of condition she is having. Can you give suggestion onto what i should do with her knee? and maybe i can attach her knee picture and send to your email?

Your wife is having considerable swelling and redness around the knee joint. The effusion - fluid collection in the knee joint is also worrisome. Damage to the internal structures of the knee is likely.
She needs proper immobilization of the joint for healing. Avoid weight bearing on the knee. If the swelling is gradually increasing then the drainage of the collected fluid and local steroid injection can help her. Do consult an orthopedician for a proper treatment.
In the meanwhile, for symptomatic relief:
1. Give enough rest to the knee- use a knee brace.
2. Take mild pain killers like acetaminophen or ibuprofen.
3. Stronger prescription NSAIDS with muscle relaxants may help if simple analgesics do not help.
4. Hot compresses to the knee will be useful pain relieving measure.

Thanks for your reply and the consultation.
I have few questions that i would want to ask you.following up to your advise.

what do you mean by damage to the internal structures of her knee? What kind of immobilization of the joint? who should i refer to for this, do you have some examples for these types of immobilization. or perhaps you can send me some videos so i could take a look at them.

what do you mean by drainage of the collected fluid? how can this drainage be done.and the same who should i refer to for this, and what kind of local steroid injection? what is the name for the drug injection.

I would like to let you know for her current condition.as follows:

* she could not bend her knees, and could not stand up for a very long time.
* she is experiencing stinging+sour pain at the right side of her knee joint.
* experiencing nerve pain at her thigh, and the lower part of her right leg.
* and it seems that the muscle at her thigh has become smaller and inwardly?
* this problem has hinder and slowed down the movement of her leg

Thanks for writing again.
The internal structures includes ligaments, meniscii and cartilage inside the knee joint. Any one of these could be torn. She needs rest and avoid weight bearing on the joint. You should consult an orthopedician for the proper treatment.
The fluid is drained with the help of a syringe by orthopedician. He will also inject steroid in the knee joint to prevent further collection of fluid inside it.
You need to consult an orthopedician, get the MRI scan of the knee done and start the proper treatment under his guidance.

Thanks for your reply. Can i ask you some more questions regarding about my wife's current condition. I am thinking of bringing her to see an orthopedician, but before i do that i just want to make sure that i am doing the right thing. As i am very worried that something would go wrong with her knee. Will she be able to walk like normal again???

What would an orthopedician usually do? yes of course she would need to get her MRI scan test first before knowing what is her actual condition and what should be done. but lets say if any of her internal structure eg: her ligament, meniscii and cartilage is torn. does she need an operation??? what would you usually do? if one of this is torn. i mean like how to get her condition back to normal again?? does she need to go through surgery and if yes.is it dangerous? because right now i think her condition is quite serious from what i see. not being able to bend and not walking like normal. has serious nerve pain which i am very worried about, also causing her muscle to become smaller inwardly. i do not understand and do not know where to start for her treatment. please.

Arthroscopic repair can be needed.

Hello.
She will be able to walk normally again with treatment. The orthopedician will order an MRI scan and if there is any major tear he will try to repair it arthroscopically. It is a minor surgery and needs admission only for a day. In case it is a partial tear then conservative treatment is tried with rest and using a brace. The injured area usually heals in 4-6 weeks.
She needs a specialist treatment.
