Suggest Treatment For TB In Lymph Nodes Pn Neck

Question: Hello doctor; I m diagnosed with tb in lymph nodes (neck).. Currently I'm taking tb medication
I m breast feeding my seven month old daughter. Please advise if the medication which I'm consuming have impact on baby through breast milk.. Shall I continue feeding

I understand your concern and situation.

As you told you have tubercular lymphadenitis. Now taking antitubercular drugs. Firstly remember regular compliance is foremost importance. Do not miss any drug, any dose so that you do not develop multi drug resistant strains of mycobacteria. The various studies have shown that antituberculosis drugs appear to be safe for use with breastfeeding. The amount of drug excreted in breast milk is relatively small concentration. Till date no adverse effects have been reported. Drugs like isoniazid, rifampin, ethambutol, streptomycin which are first line agents are compatible with breastfeeding. But still no clear data on the safety of pyrazinamide during breastfeeding. It depends on mother whether she chooses to breastfeed and examine the infant for signs and symptoms of toxicity by a paediatrician regularly. So I suggest choice can be only yours. But you must not stop treatment at any cost. Get regular follow-up and Liver function tests done. Eat healthy and protein rich balanced diet with multivitamins supplements.

Thank you so mych doctor for your answer.

When you mentioned about liver function test i want to mention that my current SGOT 42 u/l and SGPT 60 u/l (these test level before starting the tb medication course )which is above than the normal level. Is it because of tb or any other factors. Will these medication further reduce my liver function.

Hi..

Don't worry your liver function tests are normal. They are considered abnormal when they are increased at least 3 times the upper limit. ATT drugs increase these liver enzymes. So you have to get these monitored. Drink plenty of fluids. Take sweetened juices and drinks. Avoid oily and spicy food. Take liver tonics like hepamerz or scorliv twice daily.
